Position Title: Coordinated Entry & Diversion Coordinator
Reports to: Chief Programs Officer
Status: Full-Time – Exempt, In-person
The Coordinated Entry & Diversion Coordinator will increase access to Denton County’s Coordinated Entry (CE) System in rural parts of the county and for those with barriers to access. This position will support over ten agencies in Diversion and CE and increase the capacity of the county to meet the needs of households experiencing homelessness via mobile and tele-assessments.
Roles & Responsibilities:
- Administer assessments, provide supportive services and coordinate appropriate referrals following priorities defined by the Housing of Urban Development (HUD)
- Train and support county-wide Diversion & CES Assessors for phone and in-person screenings to gather information from households seeking shelter or housing
- Cofacilitate weekly case conferencing among Denton County nonprofit and social service organizations
- Assures the accuracy, completeness and confidentiality of records.
- Administers and ensures timely completion of the necessary assessment to all individuals served.
- Maintains current, accurate and comprehensive client data in each record to include all activities, contacts, relevant documents, and communications on behalf of individual served.
- Maintains all records required for the completion of monthly reports.
- Provides supportive services in accordance with program guidelines and requirements.
- Conducts daily/weekly rounds in designated areas where unsheltered individuals congregate; Makes outreach services known and offers coordinated entry intake services.
- Screens applicants, administers assessments, and makes referrals to community agencies. Links individuals with appropriate services, arranges emergency and long-term placements, and provides crisis interventions as needed.
- Evaluates effectiveness and satisfaction in following service recommendations
- Collaborate with UWDC’s Landlord Engagement to quickly prepare and move people into housing
- Collaborate with local government, law enforcement, fire and emergency medical service teams to identify and provide access to diversion & coordinated entry for people living unsheltered throughout the county
- Support continuous improvement of Denton County’s Diversion & CE System
- Intern program support: recruit and train interns to expand CE system support
- Build intern program to expand Diversion & CES Assessors available in the county
- Other duties as assigned
QUALIFICATIONS:
- Relevant experience in homelessness intervention/prevention direct social service delivery or related field OR bachelor’s degree.
- Experience utilizing cloud-based database systems; experience with a local Homeless Management Information System (HMIS) and/or the ClientTrack database system a plus
- Strong written and verbal communication skills
- Must gain or have a working knowledge and utilization of community resources in Denton County
- Ability to work with diverse client population and community organizations as well as staff at all levels
- Must be willing to work a flexible schedule that includes evenings, holidays and weekends
- Ability to show patience and courtesy when working with people is a necessity
- Must be able to interact appropriately with internal and external customers, including
- Individuals receiving services, community service providers, supervisory staff and colleagues
- Work will be performed across variety of locations in Denton County; must have your own vehicle and a valid Driver’s License and automobile insurance.
